Clinic Manager Elizabeth Carroll is thrilled to announce the implementation of a new integrated care model at the Hutchinson Health Clinic.
What is an integrated care model?- Carroll explains that this model is designed to provide additional support to the patients’ of Hutchinson Health Clinic through the use of proactive care planning, strategic patient outreach and advanced-level nursing interventions, both remotely and in-person. These interventions are timely and coordinated with your entire care team. This model welcomes the addition of four Registered Nurses to our care team as well as internal reorganization of the current support team. This addition and reorganization aligns the care team to be delivering care at the top of their licenses in a collaborative and efficient manner that best fits our patient’s needs.
Why do we have it? –Carroll explains that this is not a substitution for the current great care that this team delivers, but rather an expansion that will allow for more proactive care coordination within an interdisciplinary care model. In healthcare there is a goal called the “Triple Aim.” This goal aims to improve the health of communities and improve the care experience, all the while reducing costs by targeting complex patient needs in a proactive approach. With more resources and realignment of current resources, our team will be able to target the Triple Aim for our community in a different way than we have before.
