Diabetes Education Type 2 Group Classes

Living with diabetes can be overwhelming , but our staff is here to support you through this process. It is a lifelong disease, but it can be managed and monitored.

  • Understand your condition and work towards improving your health
  • Prevent complications or long term side effects
  • Monitor blood glucose levels
  • Plan healthy meals and snacks
  • Learn to use and manage insulin
  • Incorporate physical activities

Session A

  • Emotional health and support system
  • Review types of diabetes, and the pathophysiology
  • Diagnosis criteria, A1C testing
  • Risk factors, complications of diabetes
  • Treatment methods
  • Blood glucose monitoring and targets
  • Eating for better health
  • My plate
  • Discuss dining out strategies

Session B

  • A1C testing, meters and advanced features
  • Stress and activity, how they impact blood glucose
  • Signs and symptoms of hypoglycemia and hyperglycemia as well as causes, treatment and prevention
  • Initiation of personal goal plans
  • Sick day management
  • Meal plan including carbohydrate counting and reading food labels

Session C

  • Discuss the natural changes of diabetes
  • Develop problem solving skills
  • Long term complications associated with elevated blood glucose
  • Review carbohydrate counting as needed
  • Instruct on high cholesterol including fats and sodium for heart health
  • Weight management
  • Alcohol consumption
  • Review of food records and label reading
  • Mediterranean way of living

  • Review screening and tests for chronic complications
  • Review good food habits, food records, food labels and review screening tests for chronic complication
